Dr. Bradley K. Vaughn is a board certified orthopaedic surgeon experienced in adult total joint replacement and revision surgery.

Approach

Total hip replacement surgery (THR’s) has been one of the most proven advancements in orthopaedic history. There are various approaches used for this procedure. The most popular is the posterior approach is used in the majority of the hip replacements performed in the United States. It involves accessing the hip joint via an incision made close to the buttocks. It provides a good view of the hip capsule for optimum placement of implants. Over the years this procedure has been refined so that it is more minimally invasive, reducing the size of the incision while maintaining the safety and effectiveness of the technique. It has helped to reduce trauma, reduce the length of hospital stay, and limit adverse reactions to anesthesia.

Anterior approach has recently emerged as a viable alternative to the more popular posterior approach. It has been used to some degree since the 1980s but new instrumentation has allowed it to be performed through smaller incisions making it increasingly sought after. It involves making an incision in the front rather than the back which is the entry point for more conventional posterior hip replacement surgery. Frontal entry makes it possible to reach the joint by separating rather than cutting and reattaching muscles. It allows for a good view of the hip socket and preserves the muscles. Fluoroscopy is used during the procedure to assess placement and leg lengths.

The anterior approach is relatively new in its new form so it is important for patients to find an experienced surgeon to perform this procedure. According to Dr. Bradley Vaughn, “Anterior approach has gained popularity. In the properly selected patient, it can afford a quicker recovery.” The decision on the type of approach, anterior vs. posterior, is usually based on anatomy, bone quality, and patient weight as well as age.

The results of total hip arthroplasty are outstanding in terms of relief of pain and improvement in function. There has been to date no clinical study demonstrating the superiority of one surgical approach over the other. The possible short term benefits of one surgical approach over another must always be balanced with the possible untoward effects unique to that approach.
